The name for this system sounds complicated at first – the hypothalamic–pituitary–gonadal axis, or HPG axis – but the basic idea is simple: it’s how the brain tells the body when to produce reproductive hormones.

As the diagram shows, the process begins in the hypothalamus, a small region of the brain that releases a hormone called Gonadotropin-Releasing Hormone (GnRH).

GnRH signals the pituitary gland to release two key reproductive hormones:

  • Luteinizing Hormone (LH)
    Follicle-Stimulating Hormone (FSH)

These hormones then act on the reproductive organs.

In women, LH and FSH regulate ovulation and ovarian hormone production.
In men, LH stimulates testosterone production and FSH supports sperm development.

When new male patients come into the clinic, one of the first things we look at in their labs is LH and FSH. These two hormones tell an important story about how the brain is communicating with the testes.

  • LH acts on the Leydig cells, the cells responsible for producing testosterone.
  • FSH acts on the Sertoli cells, which support the development of sperm.
  • Together, these hormones help us understand how strong the brain’s signal is telling the testes to do their job.

In men, this signaling system tends to operate on a steady, day-to-day rhythm. Hormone levels rise and fall slightly throughout the day, but the overall pattern is fairly consistent on a daily basis for men. 

And again, Kisspeptin plays a role at the very beginning of this system.

By helping regulate the brain signal that releases GnRH, kisspeptin influences the hormones that follow – including LH and FSH. When the signal at the top of the system is strong and consistent, the hormones that depend on it tend to function more predictably.

A simple way to think about it is this: if the first signal in the chain becomes stronger and more consistent, the rest of the system has a better chance of working the way it’s supposed to.

Kisspeptin and Female Fertility

Female fertility operates very differently than male fertility because the hormonal system runs on a monthly cycle rather than a steady day-to-day rhythm.

Throughout that cycle, the brain sends signals that help regulate several key steps:

  • The development of an egg inside the ovary
  • The release of that egg during ovulation
  • And the rise and fall of hormones like estrogen and progesterone

For ovulation to occur, the brain must send a strong hormonal signal that triggers the release of Luteinizing Hormone (LH). This event (often called the LH surge) is what ultimately causes the ovary to release an egg.

Kisspeptin helps initiate the signals that lead to this surge.

By helping regulate the release of GnRH in the brain, kisspeptin influences the hormones that follow – including LH and FSH – which coordinate ovulation and the broader rhythm of the reproductive cycle.
